Health Resources

My name is Priscilla Chavez and I am the school nurse this year.

I am here to help your student(s) stay safe and healthy while learning, whether they are learning online at home, or in person at school.

As you know, school looks different this year due to the COVID-19 pandemic. We recognize that there is much concern and stress about how we will protect your children and ourselves during this global pandemic. I want to share some information with you about what to expect from the health office this school year.

If you or your student are COVID-19 positive and in isolation and/or quarantine, please let me or the principle know. We ask so we can contact you to support you and ensure we coordinate contactless materials distribution (delivery to your doorstep with no contact or by mail) so you don't have to leave isolation/quarantine to come to the drive by pick-up. This is also so that I can check-in with you to support your family with health information and resources.

I am listing some resources and helpful phone numbers during this time:

If you think you have COVID-19 or have questions

Call 1-855-600-3453

If you need help with child care, food, supplies or loss of income

Call 1-833-551-0518

COVID-19 testing information: or

Emotional or Mental Health Crisis

Call New Mexico Crisis Line 1-855-662-7474

Chronic Health Conditions

Please let me know if your student has a chronic health condition for which s/he will need nursing support this school year.

If your student(s) will require medication or a procedure while at school, please have their provider fill out their specific form. See Health Forms. I cannot give a medication or carry out a procedure without this signed document.  Your child's doctor/provider will have to complete these health forms every school year.

APS' Medicaid Department is available to help families enroll in Medicaid or refer for other eligible service. Please call 505-855-5261 or 505-855-9820 (Spanish speakers) for more information.

A Great Idea

It is a great idea to always keep a change of clothing in your students backpack.
